Please print out one of the permission forms provided (in PDF format) below. Fill it out and return it to the Glenmary Research Center by fax (513-874-1690) or mail (PO Box 465618, Cincinnati, OH 45246).

• For materials from Religious Congregations & Membership: 2000
• For materials from any other GRC publication.
